Listed below are the forms that are most used by PTAs. If you can’t find the form you are looking for, please go to the contact us page and send us an email naming the form that you need.
Just click on the form you need; either print, complete and mail the form to the state office or submit it via email to firstname.lastname@example.org ; or submit the form information electronically, if available. Electronic submission of information is preferred, whenever possible.

EVERY YEAR – THE BELOW FORM MUST BE SUBMITTED TO DELAWARE PTA. IT IS DUE AFTER ELECTIONS ARE HELD. WE PREFER UNITS USE ELECTRONIC FORM, BUT PAPER FORM CAN BE MAILED TO THE OFFICE.
IF YOUR PTA UNIT HAS AN OFFICER CHANGE AFTER THE ABOVE FORM HAS BEEN SUBMITTED FOR THE CURRENT SCHOOL YEAR, PLEASE COMPLETE THE BELOW FORM (ELECTRONIC STRONGLY PREFERRED) TO INDICATE THE CHANGE BEING MADE.
